What information can be accessed through a patient portal? ›

Patient portals can help you access medical records, set up appointments, communicate with your health care professional, and pay medical bills.

As of March 1, 2024, Baton Rouge General clinics transitioned to MyChart, the widely used patient portal powered by Epic's electronic health record.

The patient portal (named MyChart, not to be mistaken with Epic MyChart) was developed by Sunnybrook in 2005, had an early launch in 2006, became widely available in 2007, and later expanded to 10 other institutions [20,21].

Under Louisiana law, your health care provider owns the actual medical record. For example, if your provider maintains paper medical records, they own and have the right to keep the original record, but you have the right to see and get a copy of it.

What is a medical record number? ›

A medical record number (MRN) is a unique identifier assigned to a patient in an electronic health record (EHR), practice management, or healthcare IT system. The MRN is used to keep track of medical history, diagnoses, treatments, and other important information related to patient care.

A six-digit number or a seven-digit number can be used and divided with a hyphen into three parts, each part normally containing two digits. Within the number, the primary digits are the last two, secondary digits are the middle two and, the tertiary digits are the first two or three digits.

By comparison, modern portals provide more context to a patient's care. The portals let patients securely communicate with their physicians, pay bills, check services against what an insurance plan allows, download full medical records, order prescriptions and possibly interact with a chatbot for other services.

We examined patients' use of eight common portal features: messaging, medical advice message, lab tests/results, medication list, problem list, allergies, immunizations, and appointment details.

It is home to important patient details including their allergies, problems, history and medications. The Clinical Profile is also used for documenting vaccines, growth charts and storing legal and confidential notes.

Subjective data is information obtained from the patient and/or family members and can provide important cues about functioning and unmet needs requiring assistance. Subjective data is considered a symptom because it is something the patient reports.
